Clopidogrel Hepatic injury leading to cholestasis: case report
A 67-years-old man developed hepatic injury leading to cholestasis during antiplatelet treatment with clopidogrel. The man was admitted with a diagnosis of inferior myocardial infarction and underwent angioplasty on right coronary artery with an unspecified drug-eluting stent placement. He was discharged without any complications, on prescribed medical treatment which included clopidogrel [route and dosage not stated]. He came back 30 days later with nausea, icter and loss of appetite. Laboratory tests showed total bilirubin of 5, direct bilirubin of 4, ALT of 220, AST of 100, gamma glutamyl transpeptidase of 830 and alkaline phosphatase of 445 [units not stated]. Imaging and virologic studies were normal. Based on his examinations, drug-induced hepatic injury leading to cholestasis was suspected. Therefore, all possible culprit drugs were held stepwise, but bilirubin and the man’s symptoms did not relieve until clopidogrel was stopped. Hence, clopidogrel-related hepatic injury was considered.
