Dextranomer/hyaluronic acid Foreign body giant cell reaction that presented as a rectal submucosal nodule: case report

A 70-year-old woman developed foreign body giant cell reaction that presented as a rectal submucosal nodule following treatment with dextranomer/hyaluronic acid for fecal incontinence. The woman, who had a history of gluten intolerance, colonic diverticulosis, urinary incontinence and colonic tubular adenomas, presented due to change in bowel habits (fecal incontinence and diarrhoea). A colonoscopy showed a rectal submucosal nodule. She underwent an endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) of a submucosal nodule after a nondiagnostic rectal biopsy. A presumed diagnosis of necrotic neoplasm was made following an EUS-FNA. Upon a follow-up visit, she reported that she had received injection of dextranomer/hyaluronic acid [Solesta; route and dosage not stated] 5 years before the presentation. Based on this new information, a re-review of the FNA material was considered, and a correct diagnosis of dextranomer/hyaluronic acid-associated foreign body giant cell reaction that presented as a rectal submucosal nodule was made [time to reaction onset and outcome not stated].
