Femoral insufficiency fractures: 2 case reports Two women developed femoral insufficiency fractures associated with alendronic acid, or alendronic acid and ibandronic acid [indications not stated]. A 68-year-old woman was receiving oral alendronic acid 70mg once weekly for 3 years. She presented with a minimally displaced subtrochanteric fracture following a minor fall. She had been experiencing prodromal pain for 3 months, which had increased after the fall. X-ray revealed a widened lateral cortex in the subtrochanteric region and a transverse fracture line within the cortical thickening zone. She underwent an operation with intramedullary nailing, and postoperative healing was uneventful. A 73-year-old woman was initially receiving oral alendronic 70mg once weekly for 4 years, which was switched to IV ibandronic acid 3mg every 3 months for 2 years. She presented with a spontaneous diaphyseal fracture of the left femur. She had been experiencing prodromal thigh pain for 2 months. The fracture line was oblique, while a lateral cortex fracture was observed in an area of diffuse thickening and linear resorption that had been visible previously.
