Duodenal ulcerations: case report A 51-year-old man with hepatocellular carcinoma developed duodenal ulcerations after transcatheter arterial chemoembolisation (TACE) followed by doxorubicin [Adriamycin; route and outcome not stated]. The man, who had a history of decompensated cirrhosis secondary to chronic hepatitis C and alcohol abuse, was diagnosed with hepatocellular carcinoma. He underwent TACE, followed by infusion of doxorubicin 50mg mixed with 300 micron LC beads. One month after TACE, he presented with epigastric pain. Upper endoscopy demonstrated portal gastropathy, erosive gastritis and extensive, superficial duodenal ulcerations from the distal bulb down to the mid-part of the second portion. Author comment: "The mechanism underlying the development of gastroduodenal lesions after TACE are variable. Potential causes include flare of pre-exiting pathology, such as portal gastropathy, peptic ulcer disease, and gastritis, or are directly related to toxicity of the chemotherapeutic agent."
