Escitalopram Lack of adverse effects in a neonate following in utero and breastfeeding exposure: case report No adverse effects were reported in a neonate whose mother had taken escitalopram during pregnancy and also while breastfeeding. A 36-year-old-woman, who had been diagnosed with obsessive-compulsive-disorder, received escitalopram 30 mg/day from April 2004 to March 2005, and conceived her second baby in April 2005. Following a major depressive episode, escitalopram 20 mg/day was restarted at 24 weeks’ gestation. Because citalopram use has been linked with an increased risk of poor neonatal adaptation syndrome, her escitalopram dosage was gradually tapered to discontinuation during the final month of pregnancy; she received 15 mg/day, 10 mg/day, then 5 mg/day, before the drug was stopped. She delivered a healthy baby boy, with apgar scores of 9 and 10 at 1 and 5 minutes of life, respectively. The woman experienced a relapse of her depressive symptoms and, 15 days after delivery, while she was breastfeeding, escitalopram was restarted at a dosage of 20mg early in the morning. At follow-up when the baby was 3 months old, no adverse effects were reported by the paediatrician. Gentile S. Escitalopram late in pregnancy and while breast-feeding.
