QTc interval prolongation: case report A 14-year-old girl with a history of depression and self harm ingested ten escitalopram 20mg tablets after an argument with her mother, and then went to bed. During school the following day, she complained of light headedness and a heavy feeling in her legs. A school social worker was informed of the overdose and she was taken to hospital. Initial examination was normal, but she reported leg shakes and upper abdominal discomfort. Her sinus rhythm was normal on ECG, with a QTc interval of 450 msec. However, her QTc interval increased to 469 msec on day 2, and spontaneously normalised to 420 msec later that day. She was then transferred to a mental health facility. Author comment: "[E]scitalopram, even in modest overdose, may result in prolonged QTc interval. . . [T]he actual mechanism of possible cardiotoxicity with prolonged QTc interval in the context of escitalopram overdose is unclear." Scharko AM, et al.
