Various toxicities: 2 case reports In a case report, two patients were described, of whom an 87-year-old woman developed fentanyl intoxication, unconsciousness, snoring and low respiratory rate following fentanyl patch misuse and an 84-year-old woman developed fentanyl intoxication and unconsciousness following fentanyl patch misuse [dose and times to reactions onsets not stated; not all outcomes stated]. Case 1: The 87-year-old woman was found unconscious and snoring in her nursing home at 11:45. She was brought to an emergency department, where she was evaluated for possible causes. Cerebral event and metabolic dysfunction were ruled, and she was sent back to her nursing home. At that time, she reacted only to strong painful stimuli. At 18:00, a nurse took out her dental prosthesis. Around 21:15, a slimy fentanyl patch was found in her mouth; which had been applied to her skin 3 days previously (misuse). Thus, fentanyl intoxication via transmucosal absorption was considered as the cause of her unconsciousness and snoring. At 23:00 she was fully awake, but her respiratory rate was still low at 12/minute. Case 2: The 84-year-old woman with dementia was found unconscious in her nursing home. Later, her caregivers noticed a fentanyl patch sticking under her dental prosthesis. She did not have her fentanyl patch on her back anymore (misuse). Thus, it was likely that she had changed the position of the patch herself. Fentanyl intoxication via transmucosal absorption was considered as the cause of her unconsciousness. Leenders MEE, et al.
