Fluoxetine Hypothermia: case report

An approximately 31-year-old woman developed hypothermia following long-term use of fluoxetine for major depressive disorder [route not stated]. The woman presented to the psychiatry outpatient clinic at the age of 35 years with her family because of the control of her current psychiatric treatment and the complaints of decreased body temperature in recent months in 2019. Twenty years previously, she had complaints such as introversion, hearing sounds, decreased performance and poor success while in school, selftalk, low mood, sleep disorder and eating problems. Initially, she was treated with risperidone and biperiden followed by moclobemide. Her risperidone dose was reduced and later, amisulpride was added to her treatment course. Due to menstrual irregularity and weight gain, her medication was changed to fluoxetine 20 mg/day from 2006 along with aripiprazole. In 2015, she experienced an episode of hypothermia. The woman continued fluoxetine and in 2015, she was also diagnosed with major depressive disorder. She regularly received fluoxetine, but in 2019 at the age of 35 years, she again presented with hypothermia. Her body temperature was 34–35°C for the last 4 months. She had hypothermia at different frequencies every month for 4 months. Sometimes she was treated with heat application at home, while sometimes she underwent an intervention in the emergency department. Thus, her hypothermia was attributed to fluoxetine therapy. Therefore, fluoxetine was stopped and she was monitored. Following discontinuation of fluoxetine, her body temperature improved and no recurrence of hypothermia was noted thereafter. Karatas KS.
