Treatment failure: case report A 69-year-old woman experienced treatment failure following treatment with furosemide for heart failure. The woman, who had undergone pancreaticoduodenectomy 3 years earlier, was admitted for evaluation of chest discomfort on exertion and leg oedema (symptoms of heart failure ). She had chest discomfort on exertion and leg oedema for a few months. She had received oral flosemide 40 mg/ day for 1 week before the admission but it failed to control her heart failure symptoms. As she had pancreaticoduodenectomy and absent deep tendon reflexes on physical examination, an underlying chronic wet beriberi was suspected. No signs of Wernicke-Korsakoff syndrome were observed. The level of blood lactic acid was normal. The woman was then treated with thiamine [vitamin B1], following which a significant improvement in the underlying condition was noted.
