Lack of efficacy: case report A 32-year-old woman exhibited lack of efficacy while receiving treatment with ganciclovir [dosage and route not stated] for cytomegalovirus (CMV) infection. The woman, who had had lung transplant, received induction immunosuppressive therapy with mycophenolate mofetil, methylprednisolone and IV ciclosporin and ciclosporin on postoperative day (POD) 1. She also started receiving CMV prophylaxis with cytomegalovirus-immune-globulin. On POD 30, she developed diarrhoea, abdominal pain and lower gastrointestinal bleeding, and diagnosed with CMV colitis based on colonoscopy findings. She was treated with high-dose of ganciclovir 10 mg/kg/day. Ciclosporin was discontinued. Despite high-dose of ganciclovir, CMV viraemia continued to increase. The woman was treated with foscarnet and cytomegalovirus-immune-globulin dose was increased. Six days later, her clinical condition significantly improved.
