Delayed anaphylactic shock: case report A 17-year-old boy experienced delayed anaphylactic shock and subsequently died after receiving iohexol [Omnipark]. The boy, who had a history of primary sclerosing cholangitis and ulcerative colitis, received iohexol as the contract medium [dosage and route not stated] during angiography at 2pm on 12 February 2009. Angiography was carried out to investigate a suspicion of Buerger’s disease. He developed welts on his chest [time to reaction not stated] which were treated with hydrocortisone sodium succinate; afterwards, his condition was stable. He reported a mild headache at 18.00 hours, and again at 21.30 hours. He vomited at 22.40 hours. Respiratory alkalosis caused by hyperventilation was diagnosed, and he received treatment using the "paper bag method". He became unconscious at 23.35 hours. His condition suddenly deteriorated and resuscitation was performed, but he died at 01.14 hours on 13 February. Post-mortem investigations indicated that the cause of death was acute circulatory failure due to vascular hyperpermeability, as a result of delayed anaphylactic reaction to the contrast medium.
