Isotretinoin Ossification of the ligamentum flavum: case report

A 29-year-old man developed ossification of the ligamentum flavum (OLF) during treatment with isotretinoin for acne. The man presented with chronic intermittent thoracic back pain (inflammatory pain). His medical history was significant for acne, for which he had been receiving oral isotretinoin [dosage not stated] for the past 5 years. A clinical examination was normal, without restriction of spinal mobility. An X-ray was also unremarkable. An MRI of the thoracic spine revealed a hypointense linear mass within the ligamentum flavum. A CT scan of the thoracic spine revealed curvilinear hyperdense thickening of the ligamentum flavum (sagittal and transverse planes). Based on the clinical presentation and investigations, he was diagnosed with OLF, which was thought to be induced by isotretinoin.
