Neuropathy and myopathy: case report A 69-year-old woman developed neuropathy and myopathy during treatment with methylprednisolone and unspecified corticosteroids for pemphigus vulgaris. The woman was diagnosed with pemphigus vulgaris on the left side of the neck and bilateral buccal mucosae after radiotherapy for squamous cell carcinoma at the left buccal mucosa. She had received radiotherapy on the left side of her neck for 2 months. Her squamous cell carcinoma lesions had been cured totally with radiotherapy before the pemphigus vulgaris development, and it had not flared up since then. Topical corticosteroids [specific drug not stated] and systemic methylprednisolone 1 mg/kg were given after the diagnosis of pemphigus vulgaris. In 2 months of the treatment, the pemphigus lesions improved partially, but she reported partial loss of taste sensation and difficulty in lifting her arms, consistent with neuropathy and myopathy, respectively. Her neuropathy and myopathy were considered to be secondary to the use of corticosteroids. Therefore, the woman was scheduled to receive another adjuvant immunosuppressant. However, due to her malingancy history, new therapy was not added. She then received immune-globulin [immunoglobulin] to control the pemphigus vulgaris and to reduce corticosteroid doses. After 3 cycles of immune-globulin, a dramatic improvement in pemphigus lesions was noted. No new pemphigus lesion was noted after initiation of immune-globulin.
