Modafinil Visual hallucinations: case report A 13-year-old girl with Kleine-Levin syndrome started receiving modafinil [route not stated]. She took her first dose of modafinil 200mg around 7am one morning and returned to sleep, but awoke around 11am and developed vivid hallucinations. She reported seeing skulls on the floor of her room, and became frightened and agitated. EEG findings were consistent with cortical dysfunction. Modafinil was stopped. Her visual hallucinations became less frequent, and she became calmer during the subsequent couple of hours. Hallucinations did not recur. A brain MRI after modafinil withdrawal was normal, and a CT scan revealed decreased radioactivity in both temporal lobes and her right occipital lobe. Author comment: "Although the electroencephalographic findings during the episode of visual hallucination might indicate toxic encephalopathy, the patient did not [fulfil] the diagnosis of delirium because of preserved orientation and global cognition. Therefore, the most likely diagnosis was modafinil-associated isolated visual hallucination." Hsieh C-F, et al.
