Various toxicities: case report In a case report, one patient [age and sex not stated] was described, who developed hyperammonaemia, thrombocytopenia, acute pancreatitis and hepatotoxicity secondary to drug intoxication following ingestion of valproic acid, alcohol, diazepam and sertraline at an overdose in a suicide attempt [dosages not stated]. The patient, who attempted suicide by voluntarily ingesting overdose of valproic acid, diazepam, sertraline and alcohol [ethanol], was admitted to the hospital. Investgations showed hyperammonaemia (initially 77 µmol/L) with the maximum level on day 2 of admission at 255 µmol/L, which indicated high level of valproic acid (320 µg/mL). Drug intoxication was considered. Based on investigations, the patient was noted to have severe hyperammonaemia, thrombocytopenia, acute pancreatitis and hepatic cytolysis indicating hepatotoxicity. On day 3, the patient’s serum valproic acid and ammonia levels normalised following specific medical measures. A mild elevation of amylases showed slow recovery from day 1 to day 16 on conservative therapy. A seubsequent improvement in laboratory parametres was noted. Tudosie MS, et al.
