Oxaliplatin Allergy: case report A 41-year-old woman developed allergy while receiving oxaliplatin for colorectal cancer [dosages and durations of therapies to reaction onset not stated]. The woman started receiving oxaliplatin; 60 minutes after the start of the fifth cycle, at a flow rate of 1.16 mg/min (0.28 mg/mL), she developed itching hives on her trunk and thighs. Intradermal tests with oxaliplatin 0.01 mg/mL were positive. The woman started receiving a desensitisation protocol with oxaliplatin at a flow rate of 0.003 mg/min (0.001 mg/mL), increased to 0.75 mg/min (0.18 mg/mL) over 6 hours. She tolerated the next two cycles, and treatment was then stopped. Six years later, oxaliplatin was restarted [dosage not stated]. In the fifth cycle, the woman developed itching hives on her neck, face and trunk at an oxaliplatin flow rate of 0.11 mg/min (0.05 mg/mL). The oxaliplatin concentration and flow rate were decreased at the first doses. She successfully received the total dosage amount in 8 hours. In the sixth cycle, she developed facial, auricular and cervical erythema at a flow rate of 0.03 mg/min (0.01 mg/mL). After decreasing the concentration and flow rate of the starting doses, oxaliplatin was administered in 9 hours. Intradermal tests were positive at 0.1–0.01 mg/mL before the first four cycles, and positive at 0.001 mg/mL before the fifth and sixth cycles [patient outcome not stated]. Author comment: "Successful desensitisation to [oxaliplatin] may be followed by an increased clinical hypersensitivity to the drug." Tovar Flores V, et al.
