Patients with primary aldosteronism (PA) have a higher risk to develop cardiovascular events respect to patients with essential hypertension (EH); the diagnosis of PA gives the possibility to the clinician to cure hypertension with a specific treatment such as surgical removal of lesion in the adrenal adenomas (APA) and medical treatment in idiopathic hyperplasia (IHA). Methods. We have evaluated in patients with PA the presence of cardiac remodelling using an echocardiography study and we have studied the effects of specific treatment on these alterations, a group of EH patients was used to compare the differences and a group of healthy subject (HS) was used as a control. Patients included were 146 patients with PA (70 male, 76 female; age 52.8±12.2 years), 73 patients with EH (35 M and 38 F, age 55.6±12.4 years) and 40 HS (24 M and 16 F; age 55.7±6.1 years). Results. In patients with PA and EH were found statistically significant differences in respect to HS for the intraventricular septum (IVS) (11.2±1.8 mm, 11.2±0.9 vs 8.8±1.1 mm; p
