Antipsychotics Hypothermia: case report

A 39-year-old woman developed hypothermia while receiving lithium, levomepromazine, clonazepam, risperidone and haloperidol for type I bipolar disorder [routes not stated]. The woman was hospitalized with type I bipolar disorder. She started receiving lithium [lithium carbonate] 800 mg/day, levomepromazine 400 mg/day and clonazepam 7 mg/day. On the 55 day of admission, despite good adherence to treatment, her delusional persists, which leads to the addition of risperidone 2 mg/day. Two days later, she developed mild hypothermia (33.9°C), without cardiovascular complications. She was suspected of drug-induced hypothermia. Risperidone was changed to haloperidol 5 mg/day and the dose of levomepromazine and clonazepam was reduced, then lithium was withdrawn four days later. Despite this, on 67 day of hospitalization, the woman developed a new episode of moderate hypothermia (31.6°C), which was accompanied by altered state of consciousness (Glasgow score 9) and abnormal electrocardiogram findings (Osborn waves [J waves], prolongation of the QT interval with a corrected QT interval). She was then transferred to the ICU, and underwent orotracheal intubation and external rewarming. All psychotropic drugs were discontinued. Three days later, she was discharged with complete recovery. Haloperidol and lithium were gradually reintroduced, without any recurrences of hypothermia. Arditti R, et al.
