Monomorphic ventricular tachycardia: case report A 35-year-old woman presented with a 28-day history of pre-syncopal palpitations, that had increased in frequency and duration over the past 14 days. Thirty-four days earlier, she had started a 90-day course of IV arsenic trioxide 7.9mg daily for relapsed acute promyelocytic leukaemia. On presentation, an ECG revealed episodes of monomorphic ventricular tachycardia (VT) with normal QTc interval. She was admitted for further management, and tests revealed a serum potassium level of 4.3 mmol/L and magnesium level of 0.92 mmol/L. Arsenic trioxide was withdrawn, and she received potassium, magnesium and metoprolol. She was subsequently discharged. One month later, she had no further episodes of VT. At last follow up, she had been asymptomatic for 12 months. Author comment: "Our reported case of arsenic-induced VT demonstrates monomorphic VT which was not associated with a prolongation of the QTc or electrolyte abnormalities." Ducas RA, et al.
