BCG infection in an HIV-positive elderly patient: case report A 68-year-old HIV-positive man developed a granulomatous BCG infection of the seminal vesicle after having received BCG treatment for bladder transitional cell carcinoma. Four years prior to presentation, the man had received intravesical BCG [dosage and duration of treatment not stated]. He presented with a 3-day history of fever and abdominal pain. After 3 weeks of fever, a mass developed in his groin. An MRI of his pelvis suggested an abscess in his right seminal vesicle. A biopsy revealed caseating granulomas and acid-fast bacilli in the vesicle. Intravesical BCG was assumed to be the source. The man began receiving antituberculous therapy. Culturing of a Mycobacterium tuberculosis complex species resistant to pyrazinamide from a urine sample subsequently confirmed the diagnosis. After 6 months of therapy, the man was fully recovered.
