Costal BCG osteomyelitis in an infant: case report A 15-month-old boy developed costal BCG osteomyelitis following BCG vaccination. The boy had received a BCG vaccine [dosage not stated] at 3 months of age. He presented with a mass in his left anterior chest wall. The injection site on his left arm did not show any subcutaneous abscesses. His ultrasound exam revealed a heterogeneous mass with solid and cystic components. He received antibacterials, but the lesion grew larger. The mass was resected, and its contents consisted of whitish pus. Initially, he was diagnosed with tuberculous osteomyelitis. The boy started receiving anti-tubercular chemotherapy. He did not have a family history of tuberculosis (TB), so he received a presumptive diagnosis of BCG osteomyelitis. The cultured TB strain was sent for further evaluation, and it was identified as a BCG strain. At last follow-up, he exhibited bone healing with good clinical results.
