Torsades de pointes: case report An 84-year-old woman developed torsade de pointe following concomitant administration of sotalol for atrial fibrillation and ciprofloxacin for obstructive pyelonephritis. The woman presented to the emergency department with ncomplicated renal colic. Her medical history included chronic kidney disease, atrial fibrillation, transient ischaemic attack, arterial hypertension and sinus bradycardia. Ten years previously, she had undergone pacemaker implantation. She had been receiving sotalol 160mg twice daily [routes not stated] for atrial fibrillation. During hospitalisation, she was treated with oral ciprofloxacin 500mg twice daily for obstructive pyelonephritis, along other drugs. Afterwards, she was transferred to the urology department for monitoring. The next day, her lithiasis was complicated by obstructive pyelonephritis. She had left renal drainage, which showed improvement. Her therapy with ciprofloxacin was continued. On day 4, she had a sudden cardiac arrest. Therefore, she was diagnosed with torsade de pointe. The woman received cardiopulmonary resuscitation and electrical cardioversion, and her sinus rhythm restored. Drug interaction between ciprofloxacin and sotalol was considered as a cause for torsade de pointe. Therefore, her therapy with sotalol and ciprofloxacin were switched to bisoprolol and amoxicillin/clavulanic acid, respectively. Author comment: We present here the case of an 84-year-old female patient who presented with a torsade de pointe linked to a drug interaction between sotalol, a betablocker with class 3 arrhythmic properties known to increase the QT interval, and ciprofloxacin prescribed for obstructive pyelonephritis.
