Tonic-clonic seizure: case report A 25-year-old man developed a generalised tonic-clonic seizure secondary to an intentional duloxetine overdose. The man presented 12 hours following ingestion of 47 duloxetine 60mg tablets (2820mg). He had recently experienced an episode of major depression. Presenting symptoms included irritability, anxiety, worsening tension and blurred vision which were nonresponsive to IV diazepam. GI decontamination, comprising gastrolavage, activated charcoal and magnesium sulphate, was performed. He subsequently experienced a generalised tonic-clonic seizure 1 hour after gastrolavage (approximately 15 hours since ingestion). The seizure was successfully managed by administration of IV diazepam. The man recovered without further incident and was referred to an ICU for 36 hours’ clinical observation. He was then discharged 72 hours postadmission and prescribed diazepam 30 mg/day. No further seizures had occurred after 3 months follow-up. Pellicciari A, et al.
