Pemphigus foliaceous: case report A 15-year-old boy developed pemphigus foliaceous (PF) during treatment with erythromycin for pharyngitis [dosages and durations of treatment to reaction onset not stated]. The boy presented with a scaly and itchy papular rash on the upper limbs and chest associated with erosions. The appearance of the eruption was observed 2 weeks prior to the current presentation after treatment with oral erythromycin for pharyngitis. He also had a significant history of mild plaque psoriasis. Before admission, he had undergone several other therapies including systemic steroids and antihistamines, but the improvement was not achieved. An intraepidermal blister associated with scattered acantholytic cells was seen in punch skin biopsy. Intercellular staining with IgG and C3 was detected by the direct immunofluorescence. The indirect immunofluorescence had also shown intercellular staining pattern positive for IgG. ELISA revealed IgG serum antibodies against desmoglein (Dsg). His pathological, serological and clinical findings were consistent with the diagnosis of PF induced by erythromycin. The boy was prescribed on off-label treatment with SC etanercept 50mg once a week. Marked improvement was observed in both PF and psoriasis after 2 weeks of treatment with etanercept.
