Fentanyl Delusion of infestation: case report A 59-year-old woman developed delusions of infestation (DOI) during treatment with fentanyl for chronic back pain. The woman presented with a concern of "bugs" that could not be seen [sic]. The symptoms were suggestive of DOI. Upon anamnesis, it was revealed that she had started receiving fentanyl [dosage and route not stated] for chronic back pain one month prior to the onset of the "bugs" concern. The treatment with fentanyl was therefore discontinued. Consequently, her concern of bugs became much less fixed, however, she still experienced the concern of bugs. Author comment: "The findings from this study indicate that there is a significant relationship between the diagnosis of [delusion of infestation] and the likelihood of receiving a prescription for narcotics or stimulants."
