Lidocaine Delayed-type hypersensitivity: case report A 54-year-old woman received lidocaine 2% [route and dosage not stated] for a dental procedure. After 2 days, she developed pruritus and a deep mucocutaneous swelling in the injection site [outcome not stated]. Patch testing revealed strongly positive results with lidocaine. Skin prick, intradermal and SC challenge tests confirmed type IV hypersensitivity. Author comment: "Given its frequent use as preferred anesthetic for outpatient surgical procedure, lidocaine delayed-type hypersensitivity may be more common than generally reported and thus sometimes be overlooked or under-diagnosed."
