Pericardial diseases have been the “Cinderella” of heart diseases for decades. They have been poorly understood and studied even in medical schools, but they are not so uncommon in clinical practice, usually affecting young patients and having potential deep impact in their lives. Unfortunately, there is limited knowledge on the pathogenesis of these diseases, and management has been mainly empirical before the first clinical trials and large prospective studies in the last 5–10 years. These diseases are often the manifestation of a systemic disease or a primary disease with pericardial or myopericardial involvement, and different medical specialties may be involved (e.g. cardiology, cardiac surgery, internal medicine, nephrology, rheumatology, clinical immunology, oncology), thus further complicating the diagnostic evaluation and subsequent management. Moreover, the diagnosis of pericardial diseases is often challenging because symptoms and signs may be vague, insidious and non-specific, as well as findings from traditional diagnostic methods, such as ECG and chest x-ray. It is so not surprising that the coping with these diseases may be especially difficult for practitioners and physicians.
