Neonatal transfusion medicine is a distinct entity within transfusion medicine due to the unique features of the standards, practice, and biology surrounding these young patients. Unlike in adult blood banking, neonatal blood banking is much more of a boutique experience where products are customized to individual patients. This customization comes in the form of aliquot manufacturing, donor limitation, fresh whole blood use, and techniques including intrauterine transfusion and extracorporeal membrane oxygenation (ECMO). In addition, our neonatal patients have a unique set of disease management challenges including hemolytic disease of the newborn, newborn anemia and thrombocytopenia, and coagulopathy. Even risks for transfusion reactions may vary in neonates compared to adults with transfusion-associated circulatory overload (TACO) and hyperkalemia as a common concern for very small neonates requiring massive acute transfusions.
