Abdominal pain and hypovolaemic shock: case report A 51-year-old woman developed abdominal pain and hypovolaemic shock following senna-alexandrina abuse. The woman with a history of bulimia nervosa presented to the emergency department with abdominal pain and hypovolaemic shock secondary to senna-alexandrina [senna; route, dosage and duration of treatment to reactions onsets not stated] laxative abuse. After the presentation, plain abdomen radiography revealed air and feces inside the rectum, air-fluid levels in the transverse colon with decreased haustra. Laboratory investigation showed hyponatraemia, metabolic acidosis, leukocytosis and normal CRP. Thereafter, she was suspected to have ischaemic/toxic bowel inflammation. The woman was treated with activated charcoal and hydration therapy. Colonic oedema without vascular aetiology was confirmed by an abdominal CT scan, and she was shifted to a sub-intensive department and then ICU. In ICU, after supportive therapy, she developed a stomach ulcer, abrupt cardiac arrest and splenic artery pseudoaneurysm rupture, which was then embolised. Later, she was complicated with post-anoxic coma, pulmonary embolism and pneumonia. However, she had a second cardiac arrest and she died because of it [outcome of adverse reactions not stated].
