Coronary artery aneurysms after stent implantation: case report A 61-year-old man developed multiple coronary artery aneurysms after implantation of sirolimus-eluting stents [Cypher]. The man was found to have stenosis of his left anterior descending artery (LAD) and second diagonal artery. He underwent percutaneous coronary intervention with two sirolimus-eluting stents: 2.75 × 10mm in the LAD and 2.75 × 10mm in the diagonal branch. One year later he presented with atypical chest pain and angiography revealed a coronary artery aneurysm in the diagonal branch of the LAD. The man was treated with clopidogrel and other medical therapies, without complaint. Four years and three months after the stents were inserted chest pain recurred with dyspnoea. Repeat angiography disclosed multiple coronary artery aneurysms in the LAD and diagonal branch [outcome not stated]. Author comment: "[A] local inflammatory response to the polymer and/or the drug may cause delay in the healing of coronary arterial wall. Also the eluted dosage of [sirolimus] may play [a] role in this pathogenetic process." Ovunc K, et al.
