Convulsions following accidental oral administration in an infant: case report A 2-month-old infant [sex not stated] developed convulsions following accidental oral administration of Salvia officinalis [garden sage] oil. The infant was brought to an emergency department with persistent crying, restlessness, contractions in the hands and arms, nystagmus, vomiting and turning purple. Approximately 2 hours earlier, the mother had massaged the infant’s stomach to relieve gas and colic. She had then mistakenly orally administered a sugar spoon of garden sage oil to the infant. The garden sage oil had been prepared for topical use by a herbalist. Examination revealed tonic-clonic convulsions in the hands and arms, slight respiratory difficulty, opisthotonus, increased deep reflexes and clonus of the Achilles tendon. The infant underwent oral aspiration, and was administered oxygen, fluids and phenobarbital. The convulsions subsequently ceased. After gastric lavage and charcoal administration, the infant was admitted for observation. Twelve hours later, the deep tendon reflexes returned to normal and clonus disappeared, while the hypertonicity and opisthotonus showed significant improvement. Twenty-four hours later, the infant had completely recovered. Follow-up sleep EEG showed no evidence of convulsions. Author comment: Sage oil distilled from the sage plant yields substances that have a distinct epileptogenic character and studies have shown that they can cause symptoms of intoxication and convulsions. The fact that a sugar spoon of sage oil given by mistake orally to a child of suckling age can caused a poisoning episode that progressed to convulsions appears to confirm these studies.
