Sertraline Parkinsonism: case report A 75-year-old man developed parkinsonism during treatment with sertraline for depression. The man presented with gait disturbance which had been worsening over the past 3 months. Four months earlier he had been diagnosed with depression after presenting with symptoms including anxiety, depression, delusions and hallucinations, memory impairment and incontinence. He had begun receiving sertraline 50mg once daily [Zoloft]. On presentation, his gait was slow and unsteady with festination. His Unified Parkinson’s Disease Rating Scale score was 30 and his Mini-Mental State Examination score was 23/30. He was diagnosed with sertraline-induced parkinsonism. Sertraline was withdrawn and, after a month, the man had improved. He was later diagnosed with dementia with Lewy bodies, and became confined to a wheelchair. Author comment: It is "likely that his parkinsonism, evident at his initial visit to our clinic, was caused by sertraline because his gait improved markedly and other parkinsonian signs, although not completely, improved after the discontinuation of the drug."
