Zoledronic acid Orbital inflammation in an elderly patient: case report A 71-year-old man developed orbital inflammation during treatment with zoledronic acid for metastatic prostate cancer. The man presented with swelling of the conjunctiva and eyelids, and with pain and redness of the right eye. His medical history was notable for neurofibroma excision in the right orbit 22 years earlier. Since then, he had developed diplopia in extreme abduction and limited proptosis of the right eye. Examination revealed chemosis of the conjunctiva, mild eyelid swelling and a subconjunctival haemorrhage. His best-corrected visual acuity was 0.8 bilaterally. Exophthalmometry demonstrated 3mm proptosis of the right eye. Fat stranding was evident on CT imaging and his TSH level was decreased. His presentation coincided with the conclusion of a treatment with IV zoledronic acid [dosage not stated; duration of therapy to reaction onset not stated]. Subsequently, a bisphosphonate-triggered orbital inflammation was diagnosed. Bisphosphonate therapy was discontinued, and prednisone was initiated for 3 weeks.
