Inverted Tako-Tsubo cardiomyopathy: case report A 19-year-old woman developed inverted Tako-Tsubo cardiomyopathy after an overdose of amfetamine mixed salts [Adderall]. The woman was taken to an emergency department after ingesting 30 tablets of Adderall [amount per tablet not stated] in a suicide attempt. She complained of shortness of breath and pressure-like chest pain [time to onset not stated]. Her cardiac enzyme levels were elevated, with a creatine kinase MB level of 5.7 and a troponin level of 0.36. Echocardiography revealed severe hypokinesis of the base with a preserved apex. Cardiac angiography showed an akinetic base and a hyperkinetic apex with an ejection fraction (EF) of 35%, consistent with a diagnosis of inverted Tako-Tsubo cardiomyopathy. The woman received supportive care, and her symptoms resolved in 24 hours. Follow-up echocardiography, performed 3 days later, showed no wall motion abnormalities, and angiography confirmed the resolution of her left ventricular dysfunction, with an EF of 62%.
