Various toxicities: case report A 38-year-old woman developed generalised urticaria, dyspnoea and oedema of quinck during treatment with amoxicillin for angina, and anaphylaxis following accidental occupational exposure to benzathine benzylpenicillin at workplace [not all dosages, routes and duration of treatments to reaction onsets stated; outcomes not stated]. The woman, who had angina, received enteral amoxicillin [Clamoxyl] 3g. After 30 minutes of amoxicillin administration, she developed generalised urticaria, dyspnoea and oedema of quinck. Three months later, she presented to the occupational medicine department after a work accident 6 weeks prior. A detailed investigation revealed that one day prior to the accident, she developed headache and dizziness following manipulation of benzathine benzylpenicillin [Extencille] injection before administering it to another patient. The next day, she developed generalised urticaria and dyspnoea during manipulation of benzathine benzylpenicillin dose for the same patient. A few minutes later, she developed a stage 3 anaphylaxis reaction requiring hospital admission. The woman was treated with adrenaline, and was shifted to the ICU for further management. Based on the investigations, it was concluded that, she developed stage 3 anaphylaxis reaction following accidental occupational exposure to benzathine benzylpenicillin. Thereafter, she stopped her nursing practice for 6 weeks. Additionally, her spouse removed β-lactum drugs from the family pharmacy box. Omrane A, et al.
