Elevated gamma glutamyl transpeptidase (GGT): case report A 47-year-old man experienced increased GGT levels during treatment with atorvastatin for dyslipidaemia. The man was hospitalised due to dyslipidaemia. He had received pitavastatin for 2 months prior to the presentation. Following the presentation, examination showed high levels of total cholesterol and low density lipoprotein cholesterol. Liver function test showed normal result. His history was significant for smoking, but was free from alcohol use. Based on the evaluations, he was assessed at a mid-risk level risk as per the 2019 ECS/ESA guidelines. Therefore, his treatment was started with atorvastatin 10mg daily [route not stated]. Four weeks after treatment initiation, he presented again for blood tests. Blood test showed the following: GGT 407 U/L (ULN of 61 U/L), ALP 178 IU/L (ULN of 129 IU/L) and ALT 51 U/L (ULN of 41 U/L). He denied any recent use of antibiotics or alcohol intake. Serum creatine phosphokinase (CPK), total bilirubin and AST were found to be normal. Four days later, he was admitted as gastroenterology outpatient. At that time, serum biochemical tests showed the following: GGT 402 U/L and ALP 162 IU/L. Levels of ALT, AST, CPK, total bilirubin, direct bilirubin and indirect bilirubin were found to be normal. Abdominal color Doppler ultrasound and CT-abdomen demonstrated hepatic cysts [aetiology not stated]. After all other possible aetiologies were ruled out, a diagnoses of atorvastatin-induced increased GGT levels was made. The man’s atorvastatin treatment was stopped, and no other treatment was started foe the liver injury. On day 21 after stopping the atorvastatin, his blood tests showed a significant decrease in the GGT levels (152 U/L). After three weeks, the GGT levels normalised.
