BCG vaccine Inflammatory reaction at the injection site due to Kawasaki’s syndrome in an infant: case report An 8-month-old girl developed an inflammatory reaction at the site of a BCG vaccination scar [time to reaction onset not stated] and was subsequently diagnosed with Kawasaki’s syndrome. The girl, who had been adopted from Guatemala 6 weeks earlier, presented with fever and rash; she had received BCG vaccine [dosage not stated] at the age of 2 months. She had a 5-day history of fever and a macular rash had developed initially on her abdomen and spread to her limbs. On arrival at an emergency department, she had a temperature of 38.6°C and tachycardia (192 beats/min). She had a 1.5 × 1.5cm area of induration and erythema at the site of a her BCG vaccination scar and at the site of her Mantoux tuberculin skin test. She had bilateral scleral injection, unilateral cervical lymphadenopathy and bilateral oedema of her hands and feet. In addition, she had dry, peeling, cracking lips and an erythematous tongue. Laboratory investigations revealed the following: C-reactive protein 16.4 mg/dL, erythrocyte sedimentation rate 64 mm/h, a peripheral WBC count of 10 600 cells/mL with 46% polymorphonuclear leucocytes, platelet count 336 000 platelets/mm3, albumin 3.9 g/dL, ALT 222 U/L, AST 402 U/L and ALP 208 U/L. She was diagnosed with inflammatory reaction at the site of her BCG vaccination scar due to Kawasaki’s syndrome. She received IV immune globulin and high-dose aspirin and her fever, rash and mucosal symptoms resolved. Her erythema and induration at her BCG vaccination and tuberculin skin test sites improved over the next week along with her cervical lymphadenopathy.
