Skin necrosis after injection of crushed tablets: case report A 31-year-old man with a history of heroin addiction developed penile and scrotal skin necrosis after injecting crushed sublingual buprenorphine [Subutex; dosages not stated]. The man presented with skin necrosis and painful ulcers after 1 year of therapy with oral buprenorphine chlorhydrate. He reported that he had injected crushed buprenorphine tablets into the right inguinal fold the day before, and had immediately experienced sudden pain. Progressive worsening of his pain and skin lesions had led to hospitalisation. On examination, a black necrotic eschar was evident at his scrotum and the base of his penis, and painful fibrinous lesions were noted along the inguinal folds. A skin culture grew Streptococcus sanguinis. The man started receiving pristinamycin adn sulfadiazine, and his lesions markedly improved. However, he required surgical debridement, excision and skin grafting for treatment of a persising necrotic scrotal lesion. Kluger N, et al.
