Lack of efficacy: case report A 3-week-old female neonate exhibited lack of efficacy to calcitonin during treatment of hypercalcaemia [dosage not stated]. The neonate presented with vomiting, poor feeding and failure to thrive. She was born at 41 weeks of the gestational period with a birth weight of 3770g. She had a history of difficult vaginal delivery which required vacuum assistance at the time of her birth. Physical examination showed violaceous lesions with nodules located on her arm and back. The appearance of lesions was noted at 5 days of her life, which had resolved by the time of presentation. Laboratory parameters revealed low levels of phosphorous, elevated levels of serum calcium, undetectable amounts of parathyroid hormone and decreased levels of 25-hydroxy vitamin D. Hypercalciuria was observed in the urine studies. Renal ultrasound demonstrated bilateral medullary nephrocalcinosis. For the treatment of hypercalcaemia, in the hospital, she initially received SC calcitonin, aggressive hydration and low calcium formula. However, hypercalcaemia was refractory to treatment. The neonate started receiving pamidronic acid [pamidronate] for the treatment of hypercalcaemia. She showed resolution of symptoms with excellent weight gain. Her calcium levels were normal at 3-month follow up visit. However, the persistence of mild nephrocalcinosis was noted in the repeat ultrasound of the kidneys. Zarse E, et al.
