Colchicine Lack of efficacy: case report
A 58-year-old man exhibited lack of efficacy during treatment with colchicine for post-cardiac injury syndrome. The man, who had a history of coronary artery disease, dyslipidaemia, diabetes mellitus and hypertension, and had undergone 2-vessel off-pump coronary artery bypass surgery one month prior, was admitted with chest pain, diaphoresis, cardiac tamponade due to pericarditis, dyspnoea and elevated body temperature. Following examinations, he was diagnosed with post-cardiac injury syndrome and underwent pericardiocentesis. Then, he received treatment with diclofenac and furosemide. During hospitalisation, the cardiac effusion did not increase. He was discharged in a good condition. Aftrer one month, pericardiac effusion increased. He received prednisone and diclofenac for 4 weeks with reduction in the fluid volume. However, after discontinuation of prednisone, the pericardial fluid again increased. He was referred to the hospital for relapsing post-cardiac injury syndrome. He received treatment with colchicine [dosage and route not stated] without improvement. The combined treatment with colchicine and prednisone did not reduce the inflammation. Hence, pericardiocentesis was repeated. Then, he received colchicine, prednisone and unspecified diuretics for 6 months and showed slow pericardial fluid reduction. Jaworska-Wilczynska M, et al. Postcardiac injury syndrome.
