Syndrome of inappropriate anti-diuretic hormone secretion: case report A 62-year-old man developed syndrome of inappropriate anti-diuretic hormone secretion (SIADH) while receiving cotrimoxazole for pulmonary nocardiosis. The man was diagnosed with pulmonary nocardiosis. He started receiving oral cotrimoxazole 20 mg/kg/day in three divided doses. On the third day of the treatment, he developed hyponatraemia. After ruling out other causes of hyponatraemia, he was diagnosed with cotrimoxazole-induced SIADH. Cotrimoxazole was discontinued. Thereafter, the man’s serum sodium level normalised. Cotrimoxazole was re-initiated; however, on the day 3 of re-initiation, the man again developed hyponatraemia. Cotrimoxazole was changed to amoxicillin/clavulanic acid.
