Interstitial nephritis: case report A 21-year-old man developed interstitial nephritis following prophylactic treatment with cotrimoxazole after renal transplantation. The man received cotrimoxazole [route and dosage not stated] following surgery in addition to methylprednisolone and mycophenolate mofetil. He experienced delayed graft function and his serum creatinine reached 10 mg/dL, on post-surgery day 4, necessitating dialysis. Renal Doppler ultrasound revealed a reduced diastolic component with a resistance index of 0.85-0.88. Methylprednisolone was initiated for suspected acute tubular necrosis and/or an acute rejection episode, yet he failed to improve. The results of a renal biopsy were suggestive of acute interstitial nephritis [duration of treatment to reaction onset not clearly stated]; isolated tubular mitotic images were noted. Cotrimoxazole was withdrawn and further methylprednisolone was initiated. The man’s eosinophil and serum creatinine levels improved; his serum creatinine level reached 1.9 mg/dL after 2 months.
