Diclofenac Nicolau syndrome: case report
A 52-year-old man developed Nicolau syndrome (NS) following treatment with diclofenac for myalgia. The man presented with large painful ulcer on his left buttock. One week prior to presentation, he had received IM injection of diclofenac [diclofenac sodium; route not stated] for myalgia. Laboratory tests showed a slight increase in ESR. The physical examination revealed 11x17cm indurated violaceous plaque with necrotic crusted ulcer on left buttock on the upper outer quadrant of his left buttock. However, no benefit was observed, despite he received 2 week conservative treatment including mupirocin, local debridement of necrotic crust and dressings. The findings were consistent with NS [duration of treatment to reaction onset not stated] The man’s defect was closed with the help of split-thickness skin graft by plastic surgery clinic.
