Doxycycline Photoonycholysis: 12 case reports The Netherlands Pharmacovigilance Centre Lareb received reports of nine female patients and three male patients who developed photoonycholysis while receiving doxycycline. The patients, aged 8 to ≥ 71 years [exact ages not stated], were receiving doxycycline [route not stated] at 100mg once or twice daily [frequency not stated for some patients], 200mg once daily or 250mg twice daily for Lyme disease, pneumonia or tick bite [some indications not stated]. The patients developed photoonycholysis between several hours and 2 months after starting doxycycline. Six patients also developed other suspected drug reactions. These included photosensitivity reaction, skin discolouration, abdominal discomfort, nail discolouration and localised pain. Doxycycline was withdrawn in six patients, had been discontinued prior to ADR onset in two patients, and therapy was not changed in three patients [outcome unknown for one patient]. At last follow up, three patients had recovered, one patient was recovering, and three patients had not recovered [outcomes unknown or not reported for five patients]. Author comment: "The association between doxycycline and photo-onycholysis has been reported to Lareb 12 times and is supported by a statistically significant disproportionality in the Lareb-, WHO- and Eudravigilance database, as well as cases in the literature and a possible pharmacological mechanism".
