Oesophageal ulcers: case report A 45-year-old woman developed oesophageal ulcers during treatment with doxycycline for a purulent discharge recurrence. The woman, who had chronic granulomatous mastitis with recurrent spontaneous discharge, presented with a 2-day history of mid-chest discomfort associated with odynophagia, which had started 4 days after completion of a 10-day course of doxycycline 100mg daily [route not stated]. Doxycycline-induced oesophagitis or ulceration was suspected. Endoscopy showed mid-oesophageal ulcerations, circumferential scar and a cervical inlet patch. The woman started omeprazole, domperidone and calcium carbonate/sodium bicarbonate/sodium alginate [Gaviscon] treatment. Over the next few days, her symptoms improved. Biopsies taken during endoscopy revealed herpes simplex inclusion bodies and ischaemic changes, which were consistent with drug injury. She mentioned that she had just recovered from cold sores a few days prior to endoscopy. She remained well, with no symptom recurrence.
