Various toxicities: case report A 24-year-old man developed erectile dysfunction, loss of libido, numbness, soreness of testicles and depression during treatment with finasteride for androgenetic alopecia. The man, who was diagnosed with androgenetic alopecia 1999, started receiving finasteride [Propecia] 1 mg/day [route not stated]. After 2–5 days of finasteride initiation, he experienced total lack of sex drive, soreness of testicles and complete inability to achieve erection. He also experienced depression and had difficulty concentrating. However, his treatment with finasteride was continued considering these initial effects to be temporary. The numbness and sex drive or erectile function remained same with a little improvement in soreness of the testicles. The man’s finasteride treatment was stopped after less than one month. Subsequently, his symptoms recovered to some extent but the sexual function did not normalise. During the following years, he had persistent erectile dysfunction and loss of libido. He presented to a specialty clinic for sexual medicine in 2003, where he underwent extensive examinations, and was treated with sildenafil [Viagra] with minimal effect. There was no improvement in libido or erectile capacity. He then received citalopram and bupropion for depression but were stopped due to unspecified side effects. He then consulted to another clinic that specializes in testosterone treatment. Laboratory investigations revealed total testosterone level between 22.6 and 14.2 nmol/L. After 11 years, the erectile dysfunction and loss of libido was persistent. Traish AM, et al.
