Paradoxical bronchospasm, urticaria and periorbital oedema in a child with asthma: case report An 8-year-old girl developed paradoxical bronchospasm, urticaria and periorbital oedema during fluticasone propionate and salbutamol [albuterol] therapy for bronchial asthma. The girl, who had been taking these medications for the past four months with no adverse reactions, developed dyspnoea, audible wheezing, periorbital oedema and diffuse urticaria approximately 50 and 20 minutes after receiving fluticasone propionate metered-dose inhaler and salbutamol metered-dose inhaler, respectively [dosages not stated]. The girl’s symptoms improved with epinephrine [adrenaline], diphenhydramine and steroids. A levosalbutamol challenge was well tolerated. One week later, a trial of inhaled mometasone resulted in wheezes, urticaria and pruritus after the second dose. Mometasone was discontinued and the girl received montelukast and levosalbutamol [outcome not stated]. Kuriakose JS, et al.
