Frailty and sarcopenia have long been recognized as essential elements for prognostication in the aging population. In the recent past, an ever-increasing number of studies have explored the prevalence, pathogenesis, and robust prognostic relevance in patients with cirrhosis. This rising tide of data has already resulted in a change in how we describe our patients, the terms frailty and sarcopenia now permeating our vocabulary. As an emerging area of study in cirrhosis, some important questions have been answered, but many questions remain. How can frailty and sarcopenia be measured? How do they behave over time? What are the changes in muscle cellular and molecular biology that result in atrophy and weakness? How should the information about a patient’s frailty or sarcopenia be used to guide rehabilitation? Should a diagnosis of frailty or sarcopenia change whether patients are considered for liver transplant? Recognizing the importance of these questions, it is our distinct pleasure to bring together the efforts of an international team of experts and share with you a practical review of “the basics, the challenges,
