Development of heparin-induced thrombocytopenia antibodies?: case report A 49-year-old man tested positive for heparin-induced thrombocytopenia (HIT) antibodies after starting treatment with IV heparin.* The man, who was diagnosed with heart failure and multiple cardiac thrombi, started receiving IV heparin 36 000 U/day for anticoagulation, furosemide and antibiotics. He had stenosis in his right coronary artery, left circumflex coronary artery and left anterior descending coronary artery, and transient right hemiplegia. He had brain and spleen infarctions. Although he received heparin for 14 days, his thrombi did not decrease in size and his activated partial thromboplastin time remained at around 40 seconds. On further investigations, he tested positive for HIT antibodies [duration of treatment to reaction onset not stated]. Heparin was stopped. The man underwent thrombectomy and was subsequently treated with warfarin. [Patient outcome not stated.] * It was not clearly specified whether HIT antibodies developed after heparin initiation or if the patient already had the antibodies before heparin initiation.
