Skin rash, fever and diarrhoea in an elderly patient: case report A 77-year-old woman developed a skin rash, fever and diarrhoea while receiving imatinib [Gleevec] for chronic myeloid leukaemia. The woman started receiving daily imatinib 400mg [route not stated] and presented with profound malaise, progressive weight-loss, fever of up to 39°C, nausea, vomiting and a generalised rash 12 days later. On examination, she had patchy, papulomacular rashes involving her trunk and extremities, and was hospitalised. Imatinib was withdrawn, and the woman received vancomycin and ceftazidime. Her fever, skin rash and diarrhoea resolved after 4 days. The woman was well on follow-up 1 week later, and daily imatinib 400mg was restarted. She experienced fever, chills and a similar skin rash within hours of her first imatinib dose. The drug was permanently discontinued, and her rash again resolved. Author comment: "The current case had grade 4 toxicity including skin rashes as well as high fever, vomiting, and diarrhea leading to dehydration after ingestion of imatinib." Huang X, et al.
